We are recruiting a Community Response Worker:

Job Purpose:

To provide a wide range of wellbeing and welfare related support, information and advice to a variety of community members, partner agencies and professionals.

Key responsibilities:

To collate referrals for clients and complete initial assessment of needs

  1. To work alongside the team to provide support in accessing recovery focussed support services      (such as mental health, addiction support, family support)
  2.  In partnership with other local projects work together to meet community needs, signposting etc

To support community members with housing issues and apply for grants

  1. Acting as a liaison between community agencies, service providers and residents
  2. To promote positive, healthy lifestyle choices through information sharing
  3. To plan and deliver (both on site and within the community) a variety of community response workshops addressing identified need, such as accessing Universal Credit, applying for private and local authority housing etc (this will include people aged 16 plus)
  4. To work in conjunction with New Heights Volunteer Coordinator to recruit and support volunteers
  5. To maintain accurate client records and service data monitoring information on a weekly basis
  6. To ensure monitoring and evaluation information is collated to evidence project achievement
  7. To promote and represent New Heights and the NEST project at partner agency meetings and forums
  8. To attend supervisions, team meetings, training and staff development days
  9. To ensure Safeguarding principles are at the heart of the service and are dealt with in-line with the New Heights and Local Authority safeguarding processes
  10. To implement and adhere to New Heights Policies and Procedures at all times
  11. To carry out any other reasonable management requests to support the charity
